Howe long ago did you discover this?

because it is a child, you may want to let it ride for the five years (especially is no one has seen it)

If the passport os not more that 45 days old, you just need to send the passport back to office of origin with the incorrect passport, the birth certificate (with the amendment in regards to the spelling) and a note in regards to the spelling.

If it is less than one year, you can still use the amendment form, but you will have to send new pictures. If it is over one year, you will have to pay for a new passport.

But, seriously, they do not look all that closely as to the name, more that you are who you say you are
Reply:you will have to rectify the birth certificate (which is important for other reasons too - like school, and other official records).

Then you can go back to the post office and ask them the process to update the name in passport. They may issue a new one - this would mean that you would need a new set of photographs too.

Typically if name changes due to marriage, then DS-5504, Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement Form - would work. But in this case, you may want to check up http://travel.state.gov/passport/fri/Cha...

and see if you have to submit a renewal form.
Reply:Both these answers seem good but I would like to add make copies to file and keep of alll contacts regarding this matter in case it shows up somewhere else somehow. Paper trails can go in all directions now days

My son went back to his birth name years ago and I found another important document in other name., a policy deducted through bank with the name he uses now..
